However, a recent Taiwanese registry study challenges this picture: people with BPLS have a significantly increased risk of injury over many years\u2014regardless of whether they were treated or not. <\/p>\n\n\n\n<p>\ud83d\udd17 Mao JJ, Lin HC, Lin ST, et al. Incidence of Subsequent Injuries Associated with a New Diagnosis of Benign Paroxysmal Positional Vertigo and Effects of Treatment: A Nationwide Cohort Study. J Clin Med. 2024;13(15):4561.<\/p>\n\n\n\n<p><strong><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39124827\/\" data-type=\"link\" data-id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39124827\/\" target=\"_blank\" rel=\"noreferrer noopener\">Original study on <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40852519\/\" target=\"_blank\" rel=\"noreferrer noopener\"><\/a>PubMed<\/a><\/strong><\/p>\n\n\n\n<p><strong>Study design: 18 years of observation, over 250,000 individuals<\/strong><\/p>\n\n\n\n<p>The authors evaluated the Taiwanese health insurance database (LHID2005) and identified 50,675 patients with a confirmed BPLS diagnosis (at least 3 documented diagnoses by ENT specialists or neurologists). These were compared to 202,700 control subjects matched by age, gender, and comorbidities without BPLS. The average follow-up period was nearly 15 years.<\/p>\n\n\n\n<p><strong>The central findings<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>By the end of the observation period, 26.1 % in the BPLS group had been injured, compared with 17.0 % in the control group.<\/li>\n\n\n\n<li>After adjustment for age, sex, place of residence, level of care, and comorbidities, the adjusted hazard ratio (aHR) was <strong>2,63<\/strong> (95 %-KI: 2.49\u20132.88) for injuries of any kind.<\/li>\n\n\n\n<li>The most affected were <strong>Falls<\/strong> (aHR 3.44) and <strong>Traffic accidents<\/strong> (aHR 3.16) \u2013 presumably because turning the head while driving can trigger an acute attack.<\/li>\n\n\n\n<li>Also <strong>intentional injuries<\/strong> (e.g., suicide attempts, aHR 1.15) occurred more frequently \u2013 an indication of the psychological burden caused by recurrent vertigo.<\/li>\n\n\n\n<li>A clear one emerged <strong>dose-response relationship<\/strong>The more frequently BPPV was diagnosed (\u22651, \u22652, \u22653 episodes), the higher the risk of injury (aHR 2.04 \u2192 2.47 \u2192 2.63).<\/li>\n\n\n\n<li>Both minor (ISS &lt; 16) and severe injuries (ISS \u2265 16, polytrauma) were significantly increased in the BPLS group.<\/li>\n<\/ul>\n\n\n\n<p><strong>The real wake-up call: Treatment alone does not provide sufficient protection<\/strong><\/p>\n\n\n\n<p>Perhaps the most notable finding is this: the release maneuver (CRT) did not statistically significantly reduce the risk of injury (CRT: aHR 0.78, 95% % CI 0.37\u20131.29). The authors speculate, among other things, that the database did not contain information on the affected semicircular canal or the exact maneuver technique, and that some of the participants performed the maneuvers only as home exercises without professional guidance \u2014residual dizziness and unsteadiness after acute treatment could therefore continue to contribute to the risk.<\/p>\n\n\n\n<p><strong>Practical relevance<\/strong><\/p>\n\n\n\n<p>These data highlight what is often neglected in vestibular rehabilitation: purely eliminating symptoms through a repositioning maneuver does not replace structured balance training and fall prevention counseling. Especially frequent were:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Falls<\/strong> \u2013 more than 3 times as frequently as in people without BPLS<\/li>\n\n\n\n<li><strong>Traffic accidents<\/strong> - also more than 3 times as frequent, presumably because a sudden dizzy spell when turning the head in a car can be very dangerous<\/li>\n<\/ul>\n\n\n\n<p>And: The more often someone had already experienced positional vertigo, the higher the risk of injury.<\/p>\n\n\n\n<p><strong>Why doesn't the treatment help automatically?<\/strong><\/p>\n\n\n\n<p>One might assume that a successful repositioning maneuver (e.g., the Epley maneuver) would reduce this risk back to zero. However, the study shows that the risk of injury remained elevated even after treatment. One possible reason: Even after a \u201esuccessful\u201c maneuver, many people continue to experience an unsteady gait or a vague sense of unsteadiness for some time\u2014and that is precisely what continues to increase the risk of falling.<\/p>\n\n\n\n<p><strong>What you can do<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Take positional vertigo seriously \u2013 even if the individual attack only lasts seconds.<\/li>\n\n\n\n<li>After performing a recovery maneuver, do not automatically assume that your balance is fully restored.
