How vHIT and caloric testing can help with diagnosis"},"content":{"rendered":"<p><strong>For Healthcare Professionals:<\/strong><\/p>\n\n\n\n<p>Distinguishing between <a href=\"https:\/\/ivrt.de\/en\/dizziness-disorders\/menieres-disease\/\" data-type=\"post\" data-id=\"7403\" target=\"_blank\" rel=\"noreferrer noopener\">Meni\u00e8re's disease<\/a> (MD) and <a href=\"https:\/\/ivrt.de\/en\/dizziness-disorders\/vestibular-migraine\/\" data-type=\"post\" data-id=\"5313\">vestibular migraine<\/a> (VM) is one of the greatest challenges in daily clinical practice. A new large cohort study by Vosbeek et al. shows that while patient history remains the foundation, vHIT and caloric testing provide crucial pieces of the puzzle for the correct diagnosis \u2014 especially when symptoms overlap.<\/p>\n\n\n\n<p>\ud83d\udd17 Vosbeek EGM, Seelen M, Vingerhoed AM, et al. Published 2025 Aug 7.<\/p>\n\n\n\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40807145\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Original study on<\/strong><\/a><strong><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40852519\/\" target=\"_blank\" rel=\"noreferrer noopener\"> <\/a><\/strong><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40807145\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>PubMed<\/strong><\/a><\/p>\n\n\n\n<p><strong>The problem of overlapping symptoms<\/strong><\/p>\n\n\n\n<p>The study examined nearly 300 patients and confirms what clinicians often experience: symptoms are not always clearly distinguishable.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Surprisingly:<\/strong> more than 65% of VM patients experienced at least one ear-related symptom during attacks (hearing loss, tinnitus, or ear pressure), which has traditionally been associated more with MD.<\/li>\n\n\n\n<li><strong>Demographics:<\/strong> VM patients are on average younger (mean age 50 years) and much more often female (86%) than MD patients (mean age 60 years, 46% female).<\/li>\n<\/ul>\n\n\n\n<p><strong>The diagnostic power of vHIT and caloric testing<\/strong><\/p>\n\n\n\n<p>The study shows that objective vestibular function tests have a high predictive value (<strong>Positive Predictive Value, PPV<\/strong>) in distinguishing MD from VM:<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li><strong>Video Head Impulse Test (vHIT)<\/strong> An abnormal vHIT (low gain + saccades) has a PPV of <strong>81% for Meni\u00e8re\u2019s disease<\/strong>. Therefore, if the test is abnormal, MD is highly likely.<\/li>\n\n\n\n<li><strong>Caloric testing as a complement:<\/strong> Since vHIT can often be normal in MD (Negative Predictive Value only 36%), caloric testing is essential. In patients with a normal vHIT, caloric weakness with a PPV of <strong>82% indicates MD<\/strong> .<\/li>\n\n\n\n<li class=\"translation-block\"><strong>The New Threshold:<\/strong>  The authors propose increasing the cut-off value for caloric unilateral weakness to <strong>34%<\/strong>. This raises the diagnostic accuracy (PPV) for Meni\u00e8re\u2019s disease to <strong>90%<\/strong>.<\/li>\n<\/ol>\n\n\n\n<p><strong>Conflicting test results (vHIT-caloric mismatch)<\/strong><\/p>\n\n\n\n<p>A typical feature of Meni\u00e8re\u2019s disease remains the divergence between tests: abnormal caloric testing despite a normal vHIT. This was observed in 56.5% of MD patients (compared with only 24.2% in VM). One possible explanation is dilation of the semicircular canals caused by endolymphatic hydrops, which primarily alters the response to thermal stimuli.<\/p>\n\n\n\n<p><strong>Conclusion for Practice<\/strong><\/p>\n\n\n\n<p>When patient history alone is not sufficient, vestibular testing provides important guidance for diagnosis:<\/p>\n\n\n\n<p class=\"translation-block\">If the <\/strong>vHIT is normal<\/strong>, <strong>caloric testing<\/strong> should definitely be performed. Meni\u00e8re\u2019s patients are somewhat older on average (around 60 years old), and men and women are affected almost equally.<\/li>\n<\/ul>\n\n\n\n<p><strong>Why are two different tests necessary?<\/strong><\/p>\n\n\n\n<p>The \u201cVideo Head Impulse Test\u201d (vHIT) using special goggles and caloric irrigation testing are often performed together. The study demonstrates how valuable the combination of both tests is:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Video Head Impulse Test (vHIT):<\/strong> If this test is abnormal, the diagnosis of Meni\u00e8re\u2019s disease is correct in <strong>81 %<\/strong> of cases.<\/li>\n\n\n\n<li><strong>Caloric testing:<\/strong> Because the vHIT can remain normal in some Meni\u00e8re\u2019s patients, caloric testing is essential. If it shows a significant weakness (over 34%), diagnostic certainty for Meni\u00e8re\u2019s disease rises to <strong>90 %<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p><strong>Why do the tests sometimes contradict each other?<\/strong><\/p>\n\n\n\n<p>It may happen that caloric testing shows weakness while the special goggles test produces completely normal results. This is not a device error!<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Typical for Meni\u00e8re\u2019s disease:<\/strong> This discrepancy occurs in more than half of Meni\u00e8re\u2019s patients.<\/li>\n\n\n\n<li><strong>The reason:<\/strong> Increased pressure in the inner ear changes the shape of the semicircular canals. This affects the response to temperature during caloric testing much more than the response to rapid head movements during the goggle test.<\/li>\n<\/ul>\n\n\n\n<p><strong>What can you do?<\/strong><\/p>\n\n\n\n<p>If you suffer from recurring vertigo and the diagnosis remains unclear, specifically ask for both tests (vHIT and caloric testing). Only the combination of both results provides the full picture and helps determine the right treatment for you.
